Meredith walks into Derek's trailer, finding him almost naked with a beautiful woman standing right beside him. She walks away, not knowing that the woman was Derek's sister, Nancy. She states that she is very disappointed on what happened between him and Addison, she also tells him that she disapproves of Derek's affair with Meredith and proceeds to calling him the "slutty" intern.
Back in the hospital, Izzie gets back to work but is under probation and was assigned to shadow Meredith for the day. She realizes that everything had changed while she was away but still she is happy to be back.
Cristina takes charge of Burke's surgeries to avoid anyone knowing about his shaky hand. Meredith deals with a burn patient who turns out to have done it to herself to avoid taking the bar exams again. George, on the other hand deals with Noelle, a patient who has two uteruses and goes into distress when Addison accidentaly broke the news to her fiance, Greg, about her infidelity.
